Partner Contracting & Start-Up Files
Below is a list of which contract documents to use depending on whether you are a new, returning, or non-fiscal partner for the upcoming 2023-24 service year.
***Please be sure to download the correct template, fill in the highlighted sections and, using Track Changes, make any changes needed. Then send it to your correct contract contact at CivicWell for review prior to finalizing the contract for signatures.***
New Partners / Fiscal Partners (do not currently have a contract for the 22-23 service year)
CA/WA Partners: Download and fill in the forms “CA:WA - 23-24 CivicSpark Service Agreement” and the “CivicSpark Partner Contact Details” page.
CO Partners: Download and fill in the forms “CO - 23-24 CivicSpark Service Agreement” and the “CivicSpark Partner Contact Details” page.
Returning Partners (you currently have a contract for the 22-23 service year)
CA/WA Partners: Download and fill in the forms “CA:WA - 23-24 CivicSpark Service Agreement_Amendment” and the “CivicSpark Partner Contact Details” page.
CO Partners: Download and fill in the forms “CO - 23-24 CivicSpark Service Agreement_Amendment” and the “CivicSpark Partner Contact Details” page.
Non-Fiscal Partner (if you are being sponsored by another agency)
Download and fill in the form “23-24 CivicSpark Non-Fiscal MOU”
